You may choose to provide or pursue the prescription from your doctor yourself. After placing your order, if you have a copy or can obtain a copy in 48 hours from your doctor, please fax to 800.691.9840. 3D has provided you with a prescription form, letter of medical necessity, and fax cover letter to our office. The forms are provided below.
If you need assisstance, please call customer service at 1.888.585.3828 and provide us with the prescribing doctor's name, phone number, and city and state during so that we may obtain a prescription on your behalf. If we haven't received a copy of your prescription before your order is processed for shipping, a customer service rep will contact you. You may also e-mail us.
